TLDR
Captaincy went to his head, wouldn't listen to anyone else in the team. Wanted to take 5th penalty Vs Bradford and missed.
In NLD Wenger practiced with defence dropping deep to stop Bale/Lennon getting space behind, the team did this all week on training because we had to play Mertesacker and Jenkinson. During the game Vermaelen decided to play a high line and both Bale and Lennon scored identical goals exposing our high line.
That was the final stray, he only played once during the remaining games of that season. Arteta was basically made captain without it being announced. We would've been happy to let him go in 2013 but would've been short in numbers defensively so kept him.
